Healthy North Coast Ltd, trading as North Coast Primary Health Network (NCPHN), is seeking proposals from suitably qualified and experienced providers to develop and deliver a Pulmonary Rehabilitation Training Program.<br/>
The purpose of this Request for Proposal (RFP) is to identify a provider with the skills and expertise to develop and deliver training and implementation workshops to small rural providers in private practice (including sole practitioners). In addition, the successful applicant will contribute expert knowledge and experience to further design the Program including the development of outcome measures (patient reported and clinical) and payment models.
Primary Care Gold Coast Limited (trading as Gold Coast Primary Health Network [GCPHN]) is seeking responses from consultancy providers to this Request for Proposal (RFP) to scope, design and provide implementation guidance regarding the development of two projects and localised methodologies for:<br/><br/>

Central and Eastern Sydney PHN (CESPHN), invites eligible applicants to apply for QI for Cancer. Please read and familiarise yourself with this Request for Proposal (RFP) Application Guidelines (PART A) prior to completing this application form. The TenderLink portal contains questions that require responses for the submission to be considered. The application should capture in Part C all the relevant financial components that are in scope for funding.<br/>

Quality improvement (QI) activities are acknowledged to be fundamental in increasing cancer screening participation rates in practice populations. QI for Cancer is designed to function as a launching pad for longer term and sustainable practice-based activities. This program should increase bowel, breast and cervical cancer screening participation rates in practice populations by identifying and reducing the number of under- screeners and never-screeners through QI and other tailored activities. Individuals with appropriate professional qualifications or experience are encouraged to apply.<br/>
The Central and Eastern Sydney catchment spans 667 square kilometres and includes Lord Howe Island and Norfolk Island (please note that this procurement does not include coaching services for Norfolk Island or Lord Howe Island). Our region stretches from Strathfield to Sutherland and as far east as Bondi, and our boundaries align with those of South Eastern Sydney Local Health District and Sydney Local Health District. <br/>
We are the second largest of the 31 primary health networks across Australia by population, with more than 1.4 million individuals residing in our region. About the RFP <br/>
QI for Cancer aims to offer sustainable and practical quality improvement assistance for practices to increase population participation in bowel, breast and cervical cancer screening programs and improve population outcomes for these diseases.<br/>
The objectives of the QI for Cancer program are to work with a minimum of 18 pre-identified regional general practices to:<br/>> Identify and support a cancer screening 'champion 'in the practice to develop and sustain the activities once initiated.<br/>> Identify the under screeners and never screeners in the practice population.<br/>> Reduce the number of under screeners and never screeners in the practice populations by increasing targeted cancer screening activities within practices.<br/>> Recognise practice team knowledge/resource and support gaps to be addressed by the CESPHN Cancer Prevention Program Team in order to achieve improved cancer-related outcomes in the longer-term (tool will be provided). <br/>> Support data extraction needs of practices jointly engaged in the SESLHD cervical screening project and QI for Cancer Program (identified by Cancer Screening Prevention Officer).<br/>> Ensure that the practice is effectively linked up to ongoing support for cancer prevention and QI activities through the Practice Support and Cancer Prevention teams at CESPHN.<br/>

CSAPHN are offering small grants to Community Groups and organisations within regions impacted by dry conditions across regional and remote South Australia. <br/>
The newly funded Commonwealth program, Empowering our Communities SA, is directed at activity to support mental health initiatives within locations and communities most affected by the drought.<br/>
The grants are available for communities to develop wellbeing activities and interventions that facilitate community engagement and participation in regions impacted due to dry conditions and drought.
